Notice of Data Incident. After initially identifying limited suspicious activity in its network on February 23, 2024, NorthBay Health initiated an internal investigation, coordinated with law enforcement on identifying any unauthorized activity, and engaged a leading forensic security firm to assist in the investigation and confirm the security of NorthBay Health's computer systems and network. The investigations revealed that an unauthorized third party gained access to certain files on NorthBay Health's computer systems between January 11, 2024, and April 1, 2024. A review of those files determined that they contained personal information. The type of information varied by individual, but may have included an individual's name as well as their date of birth, Social Security number, passport number, financial account number, medical information, biometric information, health insurance information, driver's license number, state or other government-issued identification number, username and password, and credit or debit card number, including, for some individuals, the expiration date, security code, and/or PIN. As a precautionary measure individuals should review their account statements and monitor credit reports, and if they detect any suspicious activity on an account, should notify the financial institution or company with which the account is maintained. They should also promptly report any fraudulent activity or suspected identity theft to proper law enforcement authorities, including the police and their state's attorney general. Individuals may also wish to review the tips provided by the Federal Trade Commission ("FTC") on fraud alerts, security/credit freezes and steps that they can take to avoid identity theft.
